Clinical studies with first-generation sirolimus-eluting stents (SES) and paclitaxel- eluting stents (PES) have demonstrate a dramatic reduction in restenosis rates compared with bare-metal stents (BMS)1,2, however, reintervention rates were still high in patients with complex coronary artery disease3, and an ongoing propensity for stent thrombosis (ST) beyond one year raised further concerns regarding safety issues with these devices4. Furthermore, as both of these devices were relatively high-profile stainless steel stents, they were difficult to deliver in complex anatomy, and the SES in particular was prone to a high fracture rate, contributing to late adverse events. The everolimus-eluting stent (EES), (XIENCE V, Abbott Vascular, CA, USA; also distributed as the PROMUS stent by Boston Scientific, MA, USA) is a secondgeneration dru...
The studies presented in this thesis were conducted in a population of young, non-hospitalized subjects. This opportunity was presented by the availability of a large cohort of children and young adults that had been followed up for more than seven years as part of the Epidemiologic Preventive Organization zoetermeer {EPOZ study), and of which a large body of data on blood pressure and other characteristics could be used. The basic theme of the studies was to elucidate mechanisms involved hypertension, with special electrolytes. (chapter 1) in the early pa~hogenesis emphasis on the role of of primary dietary The effect of changes in electrolyte intake in youngsters elevated blood pressure was studied in two randomized trials. with The scientific concept in evaluating the of the randomized trial is of primary importance effect of a cert...
Hospital- based surveys and the few population studies (one in West Africa and two in South Africa) on the pattern of rheumatoid arthritis in black African population groups have created the impression that the disease is rare here and generally runs a milder course. A noteworthy exception was found to be the urbanized population of Soweto, where both the prevalence and the severity of the disease among the females resembled the usual pattern observed in the industrialized countries. Since experience in the outpatient department of Scott Hospital, a mission hospital in Lesotho, seemed to point to a different picture of the disease in this country, it was decided to conduct a prospective hospital study to verify this impression
High arterial blood pressure takes a heavy toll in western populations (1 ). Its causes are still largely unknown, but its sequelae, a variety of cardiovascular and renal diseases, have been referred to as "a modern scourge" (2). High blood pressure of unknown cause, or essential hypertension, is considered by most to be a quantitative deviation from the norm (3). Thus, the study of the distribution, determinants and development of blood pressure may provide knowledge about the aetiology and pathogenesis of high blood pressure. The basic idea of this thesis is, that it is possible to learn something about the causes of essential hypertension by inquiries of the correlates and the course of blood pressure in early life, long before the consequences of high blood pressure have occurred and early enough to prevent them. From this idea the...
Often firms lack the necessary internal resources to pursue all profitable investment opportunities at their disposal. One of the most important roles of financial markets is to allocate resources from different economic agents to the firms that will better employ them, thereby enabling productive investment to take place. However, there are informational and incentive-related problems in financial markets that result in agency costs. These costs can hinder the efficient allocation of capital across the economy and, as a result, can impact economic growth. This thesis examines the mechanisms that investors and managers use to reduce the agency costs of outside financing and the impact of such costs on firms’ investment decisions and value. The first chapter shows that the voluntary disclosure of information can help overcoming the info...
Histology of the mature gastro-intestinal tract: The gastro-intestinal tract is organized in four distinct layers, which are consistent from mouth to anus. Surrounding the gastro-intestinal lumen is the mucosa, followed by the submucosa, tunica muscularis and serosa. The inner layer of any epithelial-lined hollow organ (e.g., mouth, gut, trachea, lung, etc) is called the mucosa, which itself consists of the epithelium, lamina propria and the muscularis mucosa, which forms the boundary between mucosa and submucosa. The submucosa is a connective tissue layer supporting the mucosa and allows flexibility during peristalsis. It contains a vascular plexus, which gives rise to the capillary bed of the mucosa and also a delicate nerve network, called Meissner’s plexus. The tunica muscularis consists of an inner circular layer and an outer long...
!n hoofdstuk.
The Internet has become the key medium to obtain health information for many people. This makes the Internet an attractive and increasingly used medium for the delivery of health behaviour change programs aiming to contribute to the primary prevention of chronic diseases. Although in theory Internet applications hold great promise for the delivery of health promotion and behaviour change interventions, evidence suggests that the use of such Internet interventions is disappointingly low, especially when these interventions are implemented for use by the general public. Until recently, research on Internet-delivered health behaviour change interventions primarily focused on the effect evaluation of such interventions in more or less controlled research settings. Considerably less research attention has been directed at exploring or promo...
Amyotrophic lateral sclerosis (ALS) is a devastating neurodegenerative disease that primarily afflicts motor neurons, leading to paralysis and death within 3 to 5 years after diagnosis. Genetic studies have uncovered multiple genetic defects causing familial forms of ALS and mutations occurring in proteins with a variety of functions, including RNA metabolism (TDP43, FUS) and oxygen free radical homeostasis (SOD1). A commonality among all ALS forms is the presence of intracellular inclusions that primarily consist of insoluble protein aggregates. These inclusions indicate that protein aggregation is a central pathogenic event shared by multiple ALS forms. The research of the present thesis focuses on the role of protein aggregation and inclusion formation in two types of ALS: 1) ALS caused by mutations in the superoxide dismutase 1 (SO...
